A 31-year-old woman was working as an arborist when she stepped into a beehive and became unresponsive.
She was brought to the ED where she remained unresponsive and was intubated. Initial diagnostics did not identify signs of acute trauma. Initial MRI of brain completed and unremarkable. Her mental status gradually improved. By the time of discharge, 2 months from DOI, she was conversant and ambulatory, but with undefined cognitive challenges, as well as undefined and inconsistent visual impairment. She was recommended for inpatient rehabilitation, but inpatient rehab and skilled nursing facilities declined to take her due to level of function. She was ultimately discharged home to her family with 24-hour assistance. To complicate things, patient’s parents are divorced and each acquired legal counsel to navigate decision-making.
Patient is conversational and ambulatory, although attempts at objective assessment are impacted by impaired vision and comprehension, apraxia, and inconsistent ability to follow verbal, visual, and tactile cues. She requires stand-by assist for all ADLs.
